Have you ever had butterflies in your stomach before something nerve-wracking? Felt nauseous in the middle of a conflict? Noticed your appetite completely disappear when you’re overwhelmed or stressed?

That’s not a coincidence. That’s your gut and your brain doing exactly what they’re designed to do: talk to each other!

The connection between your gut and your brain is one of the most fascinating areas of emerging research in medicine and nutrition right now. And for people navigating eating disorder recovery, understanding this connection can be a powerful tool to support their recovery journey. It helps explain why eating disorder symptoms show up in your body the way they do. It helps make sense of why anxiety and digestion are so deeply intertwined. And it offers real hope for why healing the gut is part of healing the whole person.

The Gut-Brain Axis: A Two-Way Conversation

The Gut-Brain Axis is the name for the complex, bidirectional communication network that links your gastrointestinal tract and your brain. Through a combination of nerves, hormones, and immune signals, your gut and your brain are in constant contact, sending messages back and forth all day long.

This isn’t a new idea, but the science around it is evolving rapidly. Researchers are discovering that the gut does far more than digest food. It processes information. It produces neurochemicals. It has its own nervous system. It even influences how we feel emotionally! 

In fact, the gut’s own neural network, called the enteric nervous system, contains an estimated 100 to 500 million nerve cells, more than the spinal cord! It can operate independently of the brain and responds to stress, emotion, and physiological states in real time. The enteric nervous system is so sophisticated that scientists sometimes refer to the gut as the “second brain”.

And at the center of this entire communication system? The vagus nerve.

The vagus nerve: the star of the show

We talked about the vagus nerve in Part 2 of this series as it relates to digestion and purging. But it deserves another mention here, through a different lens.

The vagus nerve is the primary physical pathway of the gut-brain axis. It runs from the brainstem all the way down through the chest and abdomen, connecting to the heart, lungs, and the entire digestive tract. Critically, about 80 to 90% of the signals traveling along the vagus nerve go from the gut UP to the brain, not the other way around. Your gut is actually talking to your brain far more than your brain is talking to your gut.

This means that the state of your gut directly influences your mental and emotional state. A dysregulated, struggling gut sends distress signals upward. A gut that is healing and well-nourished sends signals of safety. The direction of communication matters enormously when we talk about recovery.

Your Nervous System and Digestion: Why Recovery Feels Like a Threat

Your autonomic nervous system has two primary modes, and only one of them supports digestion.

The sympathetic nervous system is your “fight or flight” mode. When it’s activated, your body is preparing for danger. Heart rate increases. Blood flow is redirected to your muscles and away from your digestive organs. Stress hormones like cortisol and adrenaline flood your system. Digestion slows or stops, because in a survival situation, breaking down lunch is not a priority.

The parasympathetic nervous system is your “rest and digest” mode. This is where digestion is designed to happen. Heart rate slows. Blood flows back to your gut. Digestive enzymes are released. The stomach contracts and churns. Nutrient absorption kicks in.

The problem in eating disorder recovery is this: eating can feel genuinely threatening to the nervous system. Not because it is dangerous, but because the body has learned to associate food, mealtimes, and fullness with fear, anxiety, guilt, or shame. And the nervous system doesn’t distinguish between a real threat and a perceived one. It responds the same way regardless. When I’m describing this to clients, I often liken it to sitting across the table from a grizzly bear. Your eyes might see a plate of food, but your nervous system is responding as if you’re about to run away from a grizzly! 

So when you sit down to a meal and your anxiety spikes, you’re not being dramatic or irrational. Your sympathetic nervous system has been activated, and your body is doing exactly what it’s programmed to do in that state. Digestion becomes harder. Fullness and bloating feel more intense. Nausea may appear. And those physical symptoms then feed back into more anxiety, making the next meal feel even harder.

This is the cycle that so many people in recovery describe –  a physiological feedback loop between your nervous system and your gut.

Visceral hypersensitivity: when the gut becomes over-alert

One of the more recent and important findings in gut-brain research is the concept of visceral hypersensitivity: a state where the nerves lining the gut become heightened in their sensitivity to normal sensations like fullness, gas, or movement.

Research suggests that chronic stress, malnutrition, and disrupted gut microbiome diversity can all contribute to visceral hypersensitivity. In practical terms, this means that the physical sensations of eating, which might feel mild or unremarkable to someone without this sensitisation, can feel intense, uncomfortable, and even painful to someone in eating disorder recovery.

This is an important piece of the puzzle, because it helps explain why recovery can feel physically harder than it “should.” Your gut’s alarm system is working overtime. Normal digestive sensations are being amplified. There are real neurobiological pieces at play here, it’s not “all in your head!”.

The good news is that visceral hypersensitivity improves as the gut heals, stress is reduced, and consistent nourishment allows the nervous system to gradually feel safer around food.

Your Microbiome and Your Mood: The Connection Is Real

We introduced the gut microbiome in Part 1 of this series through the lens of digestion and bloating. But the microbiome’s role in mental health and emotional regulation is a whole other dimension of the story worth understanding.

Your gut microbiota (the trillions of bacteria, fungi, and other microorganisms living in your digestive tract) produce and regulate a remarkable array of neurochemicals. This includes approximately 90% of the body’s serotonin, a significant portion of GABA (a calming neurotransmitter), and dozens of other compounds that influence mood, cognition, sleep, and stress response.

When gut microbiome diversity is low (which can be common in people with anorexia nervosa), this neurochemical production is disrupted. Less serotonin. Less GABA. More inflammation. Altered stress reactivity. These changes don’t just affect digestion. They affect how a person feels emotionally, and potentially how they experience anxiety, depression, and the fear that so often accompanies eating in recovery.

There is now emerging evidence that the gut microbiome may play a role not just in the physical symptoms of eating disorders, but in their development and perpetuation as well. The research in this area is still growing, but the implications are significant: healing the gut is not separate from healing the mind. They are the same process.

What this means for recovery

As you consistently nourish your body, your microbiome begins to diversify and rebuild. More diverse bacteria means more neurochemical production. More serotonin. More GABA. A gradually calmer, more regulated nervous system. Food, in this sense, is not just fuel for your muscles and organs. It is literally building the neurochemical foundation for emotional regulation.

This is one of the most powerful reframes I offer my clients: every meal is an act of emotional healing as much as physical healing. You cannot separate the two.

Supporting Your Nervous System Around Meals

Understanding the gut-brain connection is one thing. But what can it look like in practice to support your nervous system through the hard work of recovery? Here are a few evidence-informed ideas worth exploring with your treatment team.

Eat consistently and regularly

The single most powerful thing you can do for your gut-brain axis in recovery is eat consistently. Regular, adequate nourishment reduces the physiological stress signals being sent to the brain, gradually teaches the nervous system that food is safe, and provides the raw materials the gut microbiome needs to diversify and produce mood-regulating neurochemicals. Everything else on this list matters, but nothing replaces this.

Slow down around mealtimes where possible

Eating quickly in a dysregulated state keeps the sympathetic nervous system activated. Where it feels accessible, creating small pockets of calm around meals, even just taking a few slow breaths before eating, can help shift toward that “rest and digest” parasympathetic state where digestion functions best. This isn’t always possible in recovery, and that’s okay. Think of it as a direction to move toward, with each meal an opportunity to practice your regulation skills and strategies.

Lean on co-regulation

Your nervous system is wired for connection. Eating with a trusted person, a friend, a family member, a support worker, or a dietitian, can help regulate your autonomic nervous system through the process of co-regulation, where two nervous systems in proximity influence each other toward safety. If eating alone feels harder, consider whether having support present at some meals might help.

Be patient with your gut

Visceral hypersensitivity, nervous system dysregulation, and microbiome disruption all take time to heal. The sensations you’re experiencing are real, they are explainable, and they will shift with consistent nourishment and time. Your gut is not broken. It is in the process of relearning what safe feels like!

You Are Not Imagining It

One of the things I hear most often from people in eating disorder recovery is some version of: “I know I need to eat, but my body just makes it so hard.”
